    Primary school to suspend junior classes
    *

    The Centre for Health Protection has advised WF Joseph Lee Primary School in Yuen Long to suspend classes for junior students until June 4 due to hand-foot-mouth disease. A new case was reported despite enhanced infection-control measures.

    Six Primary 1 students came down with symptoms from May 8 to 16. Of them, a seven-year-old girl was confirmed to have been infected with Enterovirus-71 on May 17. All are in stable condition.

    While the centre visited the school on May 16 and cleansing measures have been enhanced, a new case involving another seven-year-old Primary 1 student was detected on May 20. The girl was admitted to Tuen Mun Hospital and is stable.


    As there were activities that included Primary 1 and 2 classes together in the school, the centre advised it to suspend classes for such students for two weeks from tomorrow for thorough disinfection to prevent further spread.

    The centre will closely monitor the situation. Parents should pay attention to their children's health condition, and remind them to observe good personal and environmental hygiene.

    Surveillance system

    The centre today also confirmed another case of EV-71 infection involving a three-year-old boy. He came down with fever, sore throat and a rash over his hands on May 17 and went to a general outpatient clinic on May 19.

    In the Legislative Council today, Secretary for Food & Health Dr York Chow said the centre has put in place a surveillance system to closely monitor hand-foot-mouth disease and enterovirus infections in Hong Kong and neighbouring regions. It is also implementing comprehensive preventive, control and surveillance measures to protect public health.


    He noted there has been a rise in hand-foot-mouth disease cases in recent months.

    Surveillance figures in April showed the consultation rate for these cases by sentinel private doctors ranged from 1.3 to 1.9 cases per 1,000 consultations - higher than the March figures, which ranged from 0.5 to 1.5 cases per 1,000 consultations.

    Rising trend

    Up to May 21, Hong Kong has recorded 25 EV-71 infection cases, higher than the numbers in previous years. There were 12 last year, 16 in 2006, eight in 2005 and 35 in 2004. But no fatal case was ever reported. There were 41,000 hand-foot-mouth cases in the Mainland so far.

    The centre's surveillance system includes outbreak notification, laboratory surveillance and sentinel surveillance at specific clinics and childcare centres. It also maintained close communication with the Ministry of Health and the health authorities of neighbouring places.

    In Hong Kong, Coxsackie A virus and EV-71 virus are the common causative agent for hand-foot-mouth disease, causing about 75% and 20% of such outbreaks. The disease occurs in Hong Kong throughout the year and the peak is usually in summer time.

    The centre may consider advising affected schools, childcare centres or kindergartens to suspend classes for thorough cleansing and disinfection for 14 days when deemed necessary.

      Four girls have been discharged from hospital while the remainder are in stable condition.

      The centre has given health advice to the school management.

          * Thoroughly clean toys or appliances which are contaminated by nasal or oral secretions.

          Children suffering from the infection should stay at home and avoid contacting other children until they have recovered.

            Three-year-old boy with encephalitis dies

            The Centre for Health Protection (CHP) of the Department of Health received a report involving a three-year-old boy under the surveillance arrangement set up by CHP and private and public hospitals concerning suspected influenza and enterovirus infections with severe complications.

            The boy died today (May 26).

            The boy had an underlying medical condition and developed fever and rash over his hands and feet on May 22.

            He was admitted to UCH in a deteriorating condition on May 24 and died with encephalitis today.

            Initial investigations revealed that the boy had no recent travel history and the nursery he attended did not have an outbreak of similar symptoms.

            The family members of the boy had no symptoms as well.

            A CHP spokesman said that the nasopharyngeal aspirates taken from the boy showed a positive result for Respiratory Syncytial Virus (RSV) and all of his specimens were so far negative for influenza A, influenza B and enterovirus including EV71.

            "RSV is a common cause of respiratory tract infection. Common symptoms include fever, chills, headache and decreased appetite," the spokesman said.

            The disease is spread by direct contact with infectious secretions.

            The incubation period is one to 10 days and is usually five to seven days.

            To prevent respiratory tract infection, members of the public are advised to take the following measures:
            * Maintain good personal and environmental hygiene;
            * Keep hands clean and wash hands properly;
            * Wash hands when they are contaminated by respiratory secretions;
            * Cover nose and mouth while sneezing or coughing and dispose of nasal and mouth discharge properly;
            * Infected persons should avoid contact with children or those with low body resistance.

              She developed a rash with blisters over her hands and feet on May 26 and sought medical advice at Tuen Mun Hospital on the same day. She did not need to be hospitalised.

              The nursery she attended reported earlier that a total of four children, including the girl, had symptoms of hand-foot-mouth disease. Centre staff have provided health advice to the institution.

              There were 12 EV-71 cases last year, 35 in 2004, eight in 2005 and 16 in 2006.

                      A spokesman for the CHP said the cases involved three boys and a girl aged between two and 11. Laboratory tests on their throat or stool samples collected earlier showed positive result to EV71.

                      Among them, a two-year-old boy developed fever, and rash over his hands and feet on May 26. He attended private practitioners on May 29 and was admitted to Kwong Wah Hospital on May 30. He was discharged today.

                      No outbreak of Hand Foot Mouth Disease (HFMD) was reported in the kindergarten the boy attended.

                      The second case involved a three-year-old boy. He developed vesicles on palms and soles on May 27 and was admitted to the Princess Margaret Hospital. He was discharged on May 28.

                      No outbreak of HFMD was reported in the kindergarten the boy attended.

                      The third case involved a four-year-old girl who developed vesicular rash over her hands on May 28. She attended the clinic of Tuen Mun Hospital on May 29. No hospitalisation was required.

                      Nine other students of the nursery the girl attended had similar symptoms between May 23 and 28. One of them was confirmed to have EV71 infection on May 28 and reported in press release the same day.

                      CHP staff visited the school and provided health advice to the management.

                      So far, no new case was reported since the implementation of control measures at the school.

                      The last case involved an 11-year-old boy. He developed oral ulcers and rash on hands and feet on May 28 and was admitted to Tuen Mun Hospital on May 30. He was now in stable condition.

                        Hand-foot-mouth disease closes nursery

                        The Centre for Health Protection has advised Po Leung Kuk Choi Kai Yau Kindergarten-cum-Nursery in Tseung Kwan O to suspend classes for two weeks due to a hand-foot-mouth disease outbreak.

                        Eight children and a staff member came down with hand-foot-mouth disease symptoms May 2 to 13, with a two-year-old girl confirmed to have been infected with enterovirus-71 on May 19. Their condition is stable and no hospitalisation was required.


                        The school management has stepped up infection control measures but a new case involving a four-year-old girl with hand-foot-mouth disease symptoms was reported to the centre today. The girl is in stable condition.

                          The one-year-old came down with a fever, oral ulcers and a rash on May 29. He went to Tuen Mun Hospital and has been discharged. His two-year-old sister also had hand-foot-mouth disease symptoms.
                          Her nursery reported an outbreak with 10 children contracting the disease.

                          The eight-year-old boy came down with oral ulcers and a rash and was treated at Tuen Mun Hospital June 1. He did not require hospitalisation and is in stable condition. While his nine-year-old sister had hand-foot-mouth disease symptoms, their primary school did not report an outbreak.

                              The first case involved a two-year-old girl who developed oral ulcer and rash over her hands and feet on June 4. She consulted her private doctor on June 6 and is now in stable condition.

                              In the second case, a four-year-old boy was affected. He lived in Shenzhen and came to Hong Kong on May 26. He developed symptoms of fever, oral ulcers and rash over hands and feet on the second day and was admitted to Princess Margaret Hospital on June 1. He was discharged on June 3.

                              As for the third case, a girl aged four developed rash over hands and feet with oral ulcers on May 30. She attended government out-patient clinic and the Accident and Emergency Department of Princess Margaret Hospital on May 30 and 31 respectively. She has recovered without hospitalisation.

                              The kindergarten she attended reported five cases (including the girl) of Hand Foot Mouth Disease with onset of symptoms between May 24 and 30.

                              Staff of CHP had provided health advice on infection control to the school management and visited the school on June 3.

                              No new case was reported.

                              CHP's investigation is on-going.

                              There are 48 cases of EV71 infection reported to CHP so far this year.

                                The three-year-old girl developed fever and rash over her hands and feet on June 6. She consulted a private doctor on June 8 and did not require hospitalization.

                                The girl's 41-year-old father and two-year-old sister also developed symptoms of Hand-Foot-Mouth Disease (HFMD) on June 11 but required no hospitalization.

                                Four other students of the kindergarten in Tin Wan Estate the girl attended had similar symptoms between June 10 and 11.

                                CHP staff visited the school and provided health advice to the management.

                                So far, no new case has been reported since the implementation of control measures at the kindergarten.

                                The 17-year-old girl involved in the second case presented with fever, headache and vesicles on her hands and feet on June 8 and was admitted to Alice Ho Miu Ling Nethersole Hospital in Tai Po the following day. She subsequently developed symptoms compatible of encephalitis and was transferred to Princess Margaret Hospital today. She is now in serious condition.

                                Investigation revealed that she had traveled to the Mainland during the incubation period.

                                The family members of the girl had no symptoms of HFMD.

                                The school she attended did not have any HFMD outbreak.

                                There are 53 cases of EV-71 infection reported to CHP so far this year.
